To initiate the process of changing your home care provider, you should inform your current provider in writing about your decision. This can be done via email, fax, or a formal letter. You'll need to provide a notice period of 1 to 4 weeks, depending on the terms specified in your agreement with them. If you're uncertain about the required notice period, we're here to help you figure it out and can also assist you in drafting the notification letter.
Call My Aged Care on 1800 200 422 and let them know that you are changing providers under the Support at Home Program. They will reactivate your referral code, which you'll need for the transition. Please ensure you write down this code for future reference.

Call your local My Companionship team and provide us with your referral code and the date you plan to discontinue care with your current provider. One of our Client Care Managers will visit your home to set up your new services, ensuring a seamless transition under the Support at Home Program from your previous provider.​
